Flu Shot Consent forms have been sent home with students today. Please read the information sheets for the vaccines. Please sign the ADH Consent Form for the flu vaccination and sign the school district (FERPA) Consent Form. Return the consent forms to your child's school by Thursday, October 19th.
Flu Shots will be given on Thursday, October 26th starting at 1:00 p.m.
